Radiation Safety Badge - Do's and Don'ts

**Attend Radiation Safety Training prior to
requesting a badge**
- Complete the RASCAL test online
https://www.rascal.columbia.edu/new/mylogin.html & e-mail
the certificate to Radiation Safety Office Attn: Maria Taveras
at mt2516@columbia.edu OR mail it to, Radiation
Safety Office, 722 W 168th St. Allen Rosenfield Building, Room
403, New York, NY 10032.
- Pick up new badges ONLY after having
attended the lecture training & completing the RASCAL test
online.
- Return all old badges via inter-office on a timely basis.
After receiving your current badge please mail to Radiation
Safety Office (address is listed above).
- Report lost badges & fill out a lost badge form.
- Always indicate deletions on the packing list that is
enclosed with the badges.
- Inform Radiation Safety of Badge Coordinator changes
- Keep all Radiation Safety Badge reports sorted in a binder
for regulatory audits.
- Researchers: Attend annual refresher
trainings

Hospital employees: Arrange an annual refresher
for your staff with the Radiation Safety Office
- Do not share badges.
- Do not discard any badges.
- Do not use your badge at another institution and, do not
wear a badge from elsewhere at Columbia University Medical
Center.
- Do not tamper with badges.
- Do not mail badges directly to the badge company, all badges
are to be returned to the Radiation Safety Office.
- Do not make any change requests to the badge company.
- Do not return badges enclosed in the original clear wrap
package, used badges should be out of package.
- Do not return your badge with your badge holder.
